We offer a comprehensive range of services to efficiently serve your medical needs. In addition to Primary Care, we have onsite lab, physical therapy, ultrasound, x-ray and more.

Primary Care, Internal Medicine, and Preventative Care

Our physicians and Advanced Practitioners manage both acute and chronic conditions. We are your Medical Home, keeping track and managing all your health needs. We have a strong passion for providing Preventive Care as well, utilizing the best medical advancements to not just treat conditions but to help protect you from them.

On-site Physical Therapy

Quick access to physical therapy enables you to get on the road to recovery much quicker and restore you to your normal quality of life. Physical therapy can address many conditions, including: muscle/joint pain/weakness, arthritis, headaches, joint stiffness, joint replacement surgeries, even vertigo and other vestibular disorders.

X-Ray

By providing onsite X-Ray we can get answers faster, cheaper and can get the proper diagnosis and plan implemented to help our patients get on the road to recovery. We frequently work patients in and get results the same day.

Care Coordination

Care Coordination provides disease education and support. Motivation is offered to increase a patient’s self management and knowledge of their condition. Nurses work closely with you to set goals and provide one on one encouragement to meet those goals. Care coordinators will act as liaisons for you and your family after hospital discharge to assist with medications, referrals and discharge instructions. Assistance with community resources is available if indicated.

Medicare and NonMedicare Annual Wellness Visits

Most insurances, including Medicare plans, cover Annual Wellness Visits. They include a review of your medical and social history related to your health, and education/counseling about preventive services, including certain screenings, shots and referrals for other care if needed. The visit is a great way to get up to date on important screenings and talk with your provider about your family history and how to stay healthy.

Annual Wellness visits are performed annually to develop or update a personalized prevention plan based on your current health and risk factors with a comprehensive review of your medical history.

Lab Services

For patient convenience we provide routine and diagnostic blood tests in the practice. Our certified phlebotomists are available 5 days a week for blood draws. Our lab is fully equipped to meet most required blood studies.

Ultrasound

Ultrasound uses high frequency sound waves to produce images of organs and structures of the body. It is a painless and harmless test involving no radiation, and studies have revealed no adverse effects. We are able to provide abdominal, pelvic, venous and carotid ultrasounds on site. Echocardiograms are also performed.

Medication Assistance Program

We have a Medication Assistance Program available to patients that need financial help obtaining expensive medications. The program begins through contact with one of our Care Management Social Workers to see if there are any resources or programs available that you may be eligible to receive help. Ambry CARE Program (genetic testing for certain cancers)

We now offer blood testing for genetic markers that may indicate risk for developing certain types of hereditary cancers. Patients aged 25 -75 that are scheduled for a Wellness visit will receive a text and/or email prior to their visit. This message will contain a secure link that guides you to a health survey, or assessment, that asks for information about your family history. This assessment is aimed at discovering if you might be at risk for developing certain hereditary cancers. If so, we may recommend this simple blood test that will identify any genetic markers that could indicate a risk of developing certain cancers. This test is NOT included in your Wellness Visit and will be a separate charge. However, most insurances do cover this test. If you have questions about cost we’ll be happy to give you all the information you need to make an informed decision before testing.

Remote Patient Monitoring

Summit Medical Group uses WellWatch by Summit to proactively support patients with the following chronic conditions: COPD, CHF, and patients who have recently been discharged from the hospital following an ER visit or an inpatient stay. Our care team will provide proactive patient outreach via weekly text messages and/or calls to ensure proper care and education. The Summit Medical Group care team intervenes before patients’ health worsens, thereby reducing hospitalizations and improving patients’ quality of life.

Telehealth Services

We do offer telehealth appointments for the convenience of our patients. Please understand that not every condition can be handled via telehealth as some conditions require hands on assessment while other conditions/reasons may be restricted due to federal and/or state law. We are unable to refill controlled substance prescriptions via telehealth.
